Information required for registration :

• All contact information (home address, phone number, work number)
• Birth certificate for verification
• A Baptismal certificate (if you have one)
• An emergency contact, their address and phone number in case we cannot reach you
• Please download, save and email the registration form back to Father.Robinson@gscs.ca
